Daughters of Charity Services of New Orleans is committed to ensuring patient privacy. We have learned of a potential privacy issue, and want to make patients who may be impacted (those who received written notice via postal mail) aware.
Regrettably, a small segment of our patient populations’ personal information – name, date of service, name of service performed, charges associated with the service received and patient identification number – has been disclosed to an unauthorized third party.
Please note that no social security numbers, addresses, phone numbers, credit card/banking or other personal information was compromised. Please also note that no test results, visit outcomes, diagnoses or medical record notes were disclosed.
We sincerely apologize and regret that this situation has occurred. Daughters of Charity Services of New Orleans is committed to providing quality care, including protecting your personal information, and we want to assure you that we have policies and procedures to protect your privacy. We are taking necessary and appropriate steps to prevent this type of incident from occurring in the future. Specifically, we are working with our electronic medical record vendor to create further safeguards from the error that resulted in this disclosure and retraining appropriate staff members to ensure that this never occurs again. We will also be reporting this incident to the applicable authorities as is required after a disclosure of protected health information.
